The St. Camillus Direct Care Program is a membership program entitling members to all of the health care services provided by St. Camillus Urgent Care & Family Practice including minor illnesses, sports and school physicals, occupational medicine, minor surgeries, minor injuries, and chronic disease management.

In regard to chronic disease management, St. Camillus Direct Care can provide management programs for diabetes, heart disease, asthma, arthritis, hypertension, and many other chronic conditions. If your condition changes and requires more specialized care, we will refer you to an appropriate specialist healthcare provider.

The Direct Care Program is an annual contract between the healthcare provider, St. Camillus Direct Care and the member(s). Membership is $60.00 per month for an individual and $95.00 per month for a family.
